Closing the Healthcare Access Gap

Established in October 2024, LifeLong Wellness Network (NPC) is a non-profit dedicated to improving health outcomes in low-resource communities. We believe everyone deserves access to affordable, early, and equitable healthcare.

We bring care directly into communities through home- and site-based screenings, health education, and linkages to essential services. By focusing on prevention and early detection, we support individuals to manage chronic conditions before they become life-threatening, particularly in areas with limited access to consistent public healthcare.

At the heart of our model are trained youth Community Wellness Workers, local health professionals we equip through accredited and non-accredited programmes that build practical skills, support professional development, and create clear career pathways in community health.

We also serve as a health implementation partner to youth development programmes and organisations, supporting the design, delivery, and strengthening of community-based health services and youth-friendly health initiatives.

Our work is grounded in research, strong partnerships, and a commitment to strengthening the health systems communities already rely on.

We refer to our Community Health Workers as Community Wellness Workers (CWWs) to reflect our holistic approach to physical, mental, and social wellbeing. This title, inspired by youth feedback, resonates positively with the communities we serve.
